Laryngeal paralysis in dogs and tracheal collapse both obstruct the airway, but at different locations and through different mechanisms. Laryngeal paralysis blocks airflow at the larynx when weakened nerves prevent the arytenoid cartilages from opening, typically producing a raspy inspiratory stridor. Tracheal collapse narrows the windpipe itself when weakened cartilage rings flatten inward, commonly producing a dry, honking cough. Both conditions require veterinary diagnosis, and neither can be reliably identified by sound alone.

Because episodes may not occur during a clinic visit, recording the sound on your phone is one of the most useful things you can bring to the appointment. A short video that captures the breathing pattern, body posture, and gum color gives the veterinarian valuable information. While recording, prioritize your dog's safety: note whether the gums stay pink, whether breathing effort improves with rest, and whether the episode resolves on its own within a few minutes.
Coughing and noisy breathing can also result from heart disease, chronic bronchitis, or lower-airway inflammationâanother reason that a recording supports rather than replaces professional evaluation.
Laryngeal paralysis most commonly affects large and giant breed dogs over nine years of age. Labrador Retrievers, Golden Retrievers, Saint Bernards, Newfoundlands, and Great Danes are overrepresented. Most idiopathic cases in older dogs are now understood to be part of a broader neurodegenerative condition called geriatric onset laryngeal paralysis polyneuropathy (GOLPP), which may also cause hind-limb weakness and esophageal dysfunction over time. Secondary causes include hypothyroidism, neuromuscular disease, and trauma, though these are less common.
Tracheal collapse primarily affects toy and small breed dogsâYorkshire Terriers, Pomeranians, Chihuahuas, Maltese, Toy Poodles, and Miniature Schnauzers. The condition has a hereditary component related to cartilage composition, and it typically becomes symptomatic between four and fourteen years of age. Collapse can involve the cervical trachea (in the neck), the intrathoracic trachea (within the chest), or both segments.
Exceptions occur in both directions. Small dogs can develop laryngeal paralysis, and large dogs can develop tracheal collapse, especially when concurrent conditions are involved. Obesity, respiratory infections, heart disease, allergies, and airborne irritants such as smoke or strong scents can aggravate either condition. Understanding how the aging respiratory system changes provides useful context for both.
Laryngeal paralysis is diagnosed through a laryngeal examination under light sedation. The veterinarian watches the arytenoid cartilages during spontaneous breathing: normally, they swing open on each inhale. If one or both cartilages remain still or move minimally, the diagnosis is confirmed. Blood workâincluding thyroid levelsâand chest radiographs help identify contributing conditions such as hypothyroidism, aspiration pneumonia, or megaesophagus.
Tracheal collapse is often evaluated first with cervical and thoracic radiographs taken during both inspiration and expiration to reveal the location and degree of flattening. Because collapse is dynamicâthe airway narrows during specific phases of breathingâfluoroscopy (real-time radiography) can show changes across the breathing cycle that static images miss. Tracheoscopy, performed under general anesthesia, allows direct visualization of the tracheal lumen and assessment of collapse severity and extent. It is considered the most reliable diagnostic tool for this condition but requires anesthesia and is typically reserved for surgical candidates.
Both evaluations may include screening for concurrent disease. Cardiac evaluation, lower-airway assessment, and abdominal imaging help the veterinary team build a complete picture before recommending a treatment plan.
Medical management for tracheal collapse may include cough suppressants to break the coughâirritation cycle, bronchodilators to open lower airways, anti-inflammatory medications to reduce tracheal swelling, and short courses of sedatives during severe episodes. Adjustments depend on the severity of collapse and the dog's response.
Surgery for laryngeal paralysis. The most common procedure is arytenoid lateralizationâcommonly called tie-back surgeryâwhich permanently holds one arytenoid cartilage in an open position. It generally improves breathing substantially, but it carries a lifelong risk of aspiration pneumonia because the surgically opened larynx can no longer fully protect the airway during swallowing. Owners should monitor eating posture, food consistency, and any post-meal coughing closely.
Surgery for tracheal collapse. Options include extraluminal support rings placed around the outside of the trachea and intraluminal stenting, where a self-expanding metal stent is deployed inside the collapsed segment via endoscopy. Stenting provides immediate airway improvement but may lead to complications including granulation tissue formation, stent migration, or stent fracture over time. Surgical selection depends on the location and extent of collapse, the dog's overall health, and the experience of the surgical team.
